District Chorus Audition Materials and New Med Form

Below are reference recordings of the melody line and accompaniment for "Simple Gifts."  If you are auditioning for District Chorus this week, I must have your paperwork at the time of the audition.  I am available period 4, 6, and 10 Thursday, and periods 6 and 10 Friday.  I am also available after school both days.  Please email me to schedule an audition. 

Also below is a new version of the medical form for District Chorus.  If you have already filled one out for District Chorus, you do not need to fill out another.  This version allows you to type directly into the form.
