Parent Teacher Conference Form / Information

I am putting together my schedule together for the upcoming parent teacher conference day on Friday, November 8th.  If you would like to meet with me to discuss your student's progress, feel free to fill out the attached form, and I will get back to you with an exact time.  If you are unable to attend, but would still like to discuss your student's progress, I will contact you at the phone or email address provided on the form during that day.  As always, I am available through email or phone if you have any questions or concerns.
